A 48-year-old obese male (BMI 35) presents with excessive daytime somnolence, witnessed apneas, and morning headaches. Epworth Sleepiness Scale score is 18. Polysomnography shows AHI of 42 with SpO2 nadir of 79%. He is also hypertensive. What are the indications for CPAP vs BiPAP? How do we manage CPAP non-adherence, and when is mandibular advancement or surgical intervention appropriate?
